Driver's Safety Course
READ DRIVER'S SAFETY COURSE INFORMATION BEFORE FILLING OUT FORM BELOW
A.) I understand that I must present to the court the following with this request:
1. Copy of a valid Texas driver’s license or permit;
2. Copy of proof of financial responsibility pursuant to Chapter 601, Transportation Code (automobile liability insurance); Needs to show that you are a listed driver on the policy.
3. Payment of non-refundable state fees and court costs totaling $144.00 (Payable to the Cross Roads Municipal Court).
4. Submit an attestation that I am not currently taking a DSC nor have I completed a DSC within the preceding 12 months from the date of my current offense, that is not shown on my driving record as maintained by the Texas Department of Public Safety; and
B.) I understand that I must:
1. Complete a TEA approved 6- hour driving safety course as applicable within 90 days of this request;
2. Submit by the 90th day from this request a uniform certificate of course completion of a driving safety course as evidence that I have completed such a course; and
3. Submit by the 90th day from this request a copy of my driving record as maintained by the Texas Department of Public Safety.
C.) I understand that:
1. If I comply with the court order that grants the taking of a driving safety course and submit all of the required evidence as ordered, the court will dismiss my case and report to the Texas Department of Public Safety the date that I completed my course for inclusion on my driving record;
2. Failure to submit all documentation required by the court will result in a show cause hearing in order to explain to the Judge why I failed to submit the necessary items. Failure to appear at the Show Cause hearing will result in a judgment of conviction being entered for my charge.
